Community Input Forums

The Western NSW Local Health District invites you to participate in Community Input Forums it will be conducting. They will be held at several locations throughout the District to seek information from communities on:

  • The three major issues affecting the health and well-being of the people in your community.
  • Priorities to improve the health and well-being of the people in your community.
  • Gaps in health services provided in your community
  • What specific things would you like to see happen in relation to
    • Services for people with cancer
    • Services for people with heart disease
    • Aged care services.

If you are unable to attend but would like to provide input please complete our online survey by clicking on this link https://www.surveymonkey.com/s/WNSWLHD-Community_Input. A printed copy of the survey form is available from your local Health Service Manager or Health Council.

The Community Input forums will be of two hour duration and held at the following sites:
